I did come up with one job/set of jobs that just don't exist (rather than hard to transfer): Things that are specialized to the US health insurance system, like medical billing or coding.
By the way other countries have nurse practitioners, UK and Canada come to mind. The US has the most specialties though.

It's possible medical billing can be done remotely though. Someone who's in that field can look for remote positions that they could do while in Israel. Also, possibly one may be able to find a different job in Israel, that utilizes their skills and experience from their previous job.
