Do you think it's possible to toilet train 3 yr old boy who doesn't care WHATSOEVER if his diaper is extremely dirty, and will happily carry on with whatever he's doing (including eating) even if his diaper is very full of yuck? Is it possible or does a child need to care already about being dirty?

Hmmm thats a tough one.
I would train him even if he didn't seem to mind being dirty. Show him the positive of going to toilet. How much fun it is to "flush the toilet" and to wash hands etc etc. I start the first 1 or 2 weeks by giving him a smartie after every time he "goes". That works very well for me at the start.
