They do different safety tests in the US and Europe, it didn't pass or doesn't have the correct features for a US approval.
It's totally safe just not legal. It will cause problems with insurance and traffic stops as others have said.

Israel agree with both US and Europe so carseats from both countries are sold here but have to be used accordingly for it to be safe. E.g if you have an American carseat you have to use the chest clip but you can have a European one that doesn't have one.

I know people who do a lot of things . I also know people that don't buckle their children into the doona that they purchased from Europe and brought to usa. Doesn't mean I'd advise others to do it or that we should copy them
Unless you work directly with gov or doona company I don't think you have set info ad to why it's not approved in usa yet.
Many child gear companies are made different in different countries and have,different safety ratings in different places
