I think I would give a smile and a "good job" for when she goes in the toilet, and I would not focus on "accidents" at all. Just teach her to wipe up and change clothes. I don't think it's unusual at the age of 4 to be still struggling with toilet training, and the less of a deal you make of it the quicker it will normalize.

Hugs, OP. I assure you that even kids without some of the challenges your dc is facing still have accidents at 4. While your child's specific challenges might make an inquiry with the doctor helpful, I don't think in my layperson's experience that it's so unusual. I might suggest that you don't ask in front of the child so that child is not shamed. Good luck and wishing your child good health.
