You do need to discuss with a rav since it is not lechatchila. If there is a medical reason for your diet then you’ll get a heter without issue.
I eat one small roll Friday night and another shabbos day. My ketone and glycemic levels don’t seem affected by it.

I don’t count. I just eat low carb foods and those foods are mostly unprocessed.
I focus on protein, and am not afraid of fat.
Do not undereat! Do not walk around hungry! This has to be sustainable. Hunger is not sustainable.
You can use carb counts to learn about what foods are low in carbohydrates but other than that, if you’re eating low carb food, there’s no real need to count. There’s no magical number that one has to hit.

I agree with you about carbs being a big part of managing pcos effectively. Pcos responds well to an anti inflammatory diet. Dairy, gluten, and sugar are big triggers for inflammation and it's been recommended to stay off those foods to help reverse symptoms.
Personally, I've switched over my diet to a much cleaner way of eating. I've also stopped using products with chemicals that have been classified as hormone disruptors. Bh bI I've reversed a lot of pcos symptoms and have had regular cycles and gotten pregnant easily since doing this. But I still haven't lost weight.

I have challah or matzah on shabbos and yomtov when required to wash. I do my best to limit how much I eat. (I do love bread, so it's a struggle for me). I do best when I have small rolls. I limit myself to 1 roll per meal. I went low carb/high fat to reverse my diabetes. I also decided (for myself, not others) that if I am eating bread for a mitzvah, it shouldn't hurt me. And it hasn't. I'm still losing weight.
I don't check ketone levels, carb content, or calories. I do check ingredients. If I can't pronounce an ingredient, or have to look up what it is, I don't buy it.

for hamotzie on sha bbos, I get a royo roll. some jewish stores have them or you can buy the m and freeze them. they ship right to you
