For a girl you have to spread her legs, spread her lips, in a well lit room you can actually see it crawling. They look like tiny white threads. Also check her back by the rectum. Maybe Google to see what to look for. If you know what to look for and there's a active case of pinworms, it's very easy to spot especially at night.

Doc told me to check at night while child is sleeping, open diaper, and look in with a flashlight.
I went to him before when he was screaming bit I didn't see anything.
Anyone know if child has to be sleeping or they would come out even when they're up?

They come out as the child is almost asleep or very recently went to bed. You might want a flashlight or phone if the room is dark. Gently pull apart tush cheeks and you should see the worms crawling out of the backside, or already outside. Possibly further up if it's a girl. I also check their BMs regularly, in diapers or the toilet. I've caught many an undetected case that way.

If you find in one kid, no need to check the rest. Just get prescription for all siblings and parents. That’s what the doctors all say.

TMI warning...
The worms often itch when they are still quite far up, so you have to shine a light and wait for them to come out towards it, or look actually inside, if the child allows and doesn't find it uncomfortable. They can also hide in the folds of the anu s which makes them hard to see.
To be honest, if my child seems uncomfortable in the evening and I don't see anything obvious, I treat them anyway. The only reason I actually look is because if you take them away the child usually has complete relief from the itching at least until another one turns up...
