depends on how much you want to spend....... maybe matching pajamas for all the kids or a toy that the older kids can use now?

If it's the 3rd or more, then the do have everything... but it's old and use and they'll be happy for a nice brand new outfit. Don't buy tiny, rather 1y.o- since the iny stuff stays ggid anyway and you get less "big sizes" anyway. If they have more than 3 they might not have the finances to buy brand new.
For a #2, I'd get a matching set, or a set sized for the older kid (they can decide if they want to use it now or wait), a story book which could be shared by both kids when mum is reading ...
Also, if you're close enough, you can ask. maybe for #1 they didn't /couldn't buy something they would like or something broke. I for one have 2 kids but no carry-sling: a friend or mine lend me hers (my kids were born after her #2 was too big and before she got pregnant with her #3). So I know I'd love one for the day I have a baby again iy"h.

For the second girl, she may have clothes from the first, but were they born the same time of year? If the oldest was born in the summer and she just had a second in the winter, she may only have newborn summer clothes and nothing warm (ditto for 12-month clothes).

When my Sister had her #2 I bought her a gift certificate from pampers. If you go to their website they have a boutique and you can buy different options a months worth of diapers or something like that etc.. They send it in a nice travel wipes box and I think with a rattle or toy. She loved it!

New clothes is always appreciated especially if it is a different season then the others or if it is a 3rd or 4th the old clothing is probably worn already and new clothes is exciting.

I liked a nursing cover for #3 because my older children now pull off my blanket when nursing.

If the babies are born in different seasons, clothes would probably be appreciated! I have 2 sons, one born in October and one born in June. I was very appreciative of clothes for the second boy. I also have 6 girls, too. The first 5 were all born from April-October. #6 was born in February, I didn't have many really tiny winter clothes. All the other seasons have been "off" for her too. (It's fun going shopping, though!). You can always buy the two girls matching dresses (for Pesach, perjaps)! My 1st 2 are 14 months apart...I loved matching outfits for them!

About one who just had her 4th: Many baby gear items aren't meant to last through so many kids...she may need a new bouncy seat or swing. Clothes also wear out over time, so even a new cute outfit may be a good idea.

This is very true! Nothing like something to make the younger siblings feel special and keep them busy for a few hours.

One of my friends just had her third. I got her a gift certificate to a store that has clothes for all ages - baby through adult - so she can buy something for the baby if she needs, something for the older kids if she doesn't need for the baby, or something for herself if she so chooses. You could get a gift certificate to a place like the Gap, which has clothes for all, or for a place like Target, which has clothes, diapers, toys, etc, or to a place like Babies R Us which has clothes plus any other baby-related stuff that she may not have from her previous kids, that may have broken/worn out, or that may not have existed when she had her older kids.

My best friend is pregnant with her 5th. If she won't tell me what she needs/wants, I am going to get her a gift certificate for a massage because she was jealous when I won one at a Chanuka party, and goodness knows she deserves one!
