Most of the time.
There have been times when I didn’t for random reasons. In that case I leave my back door unlocked and tell them to use that. (It’s rarely locked anyways)

I’m usually hosting someone I have some connection to. A neighbors relatives for her bar mitzva. A distant cousin who needs a place for shabbos. My Torah mate, or of course my own family. Any of those people I would give my combo to, even if I don’t know them personally

The neighbors cousin is basically a random stranger I wouldn't host.

What do you mean trust enough to host. I don’t know the people I host. I host for friends simchas, I have occasionally hosted for community members simchas, and on rare occasions-I live near a hospital and have hosted family at the request of someone on the Bikur cholim committee who was approached in a hallway.
I don’t have kids home, so I don’t worry about their safety.
People also typically behave when they know they are accountable (they are xyz’s guest) and you know they are there, but I don’t trust them that I would enable them to have access to my house at their discretion. (I don’t mean what time they come and go over that Shabbos, I mean if they wanted to let themselves in a week later)
Think of it this way,I leave my cleaning lady in my house alone for 2 hours, and she knows I know she is there and the only one there, but not so sure I am giving her keys to take home and potentially share with someone else.
FYI,
I give guests keys to use over Shabbos, but I take them back. I assume they are not having copies made on Shabbos (or even Saturday night )

I have a side door by my guest room with a combination and deadbolt. When I don’t have guests in my basement the deadbolt is closed.

What I've done is unlock the door before going to bed and ask that they lock it when they come in. I generally like my doors locked but I don't feel too uneasy leaving it unlocked for a little bit, assuming it probably wouldn't be more than an hour or so between me going to bed (usually late) and them coming in (how late is your meal going anyway?)

I was going to say this - do people just have a combo without a deadbolt or another lock in addition to the combo lock? At night or when I am away I always lock the second lock (in previous apartment we had a chain too). Most of the time my front door is actually unlocked - kids coming & going during the day. But I usually give my guests the combo. If I feel uncomfortable with them not to give them the combo, how would not giving them the combo help if they are staying in the house anyway!

I hear your points.
Sometimes we host people as a chessed that aren’t the healthiest, maybe don’t have the best boundaries or whatever. I don’t want to worry that they might pop in sometime when I’m not expecting it.
Or if someone is really more unhealthy than they appear to be- who knows?
