Yes it does. Each district has their own people in charge of different tasks.

Child's name and date of birth

Did you have proof of submission of the June 1 letter?
Was the SETSS authorization that you got dated as of September 1?

I only got approved for services end of August (aged out of preschool) but yes, the setts form was dated 9/5, I believe.

Pardon my ignorance, what is proof of submission? Would email be sufficient?

I looked through my emails now and see the I signed papers In may. Will have to ask yaled again what's going on.. my provider spoke to them.. what's her number or email address? She called me for something else

Foward yeled the email that you have with proof that you signed the doc in May. I had that last year- the DOE was denying that I signed the June 1 letter, so my attorney had me send the confirmation email as proof. The DOE is playing this game now of trying to get away with whatever you can't prove. By one child’s hearing they wanted the physical copy of the providor's NY teacher’s certificate, even though it's issued by the DOE and they have access. Most providers also don't have a physical copy- just an online one. Whatever petty trick will save them a few dollars, they are trying now...
