But it was returned. In the outside box after hours. Has anyone experienced?

Yes, go to the shelf where the book would be shelved and see if it's there, I have found some "lost" books there.

Ask for amnesty, sometimes they allow a 1x amnesty.
Is it an expensive book?

Same. I've also done another check at home after feeling sure that a book was returned, and it turns out that it wasn't. Worst case, my library allows us to replace it with any book that matches the missing one. So I buy one used for $5 instead of paying their $20 fee or whatever.

Lots of experience after decades and hundreds of books borrowed!

Same! And they acted shocked and like it had never happened before in the history of the library.
