Do have to fill out juror qualification questionnaire?

I got in the mail a questionnaire if I qualify as a juror. I says that it's required by law for me to send it in. Is that true? If I don't send it in what happens? What I it got lost in the mail and I never received it?

Do you have a child under two? If you do then send in a copy of the birth certificate and you are dismissed. Been there done that.

Yes but it doesn't ask anything about why I wouldn't qualify, it just asks basic questions if I'm over 18, a ny resident etc

Why would you think the government sends out forms that says you're legally required to fill it out, but really they're lying and it's not required?

I was actually called in for jury selection and (after waiting for a looong time) they took one look at me and sent me home. I assume they were turned off by my orthodox looking attire. Lol I was actually quite disappointed at that point.

Chances are you’ll never be picked.

Look, you should fill it out. What will happen to you if you don't? Likely nothing. You'll get another one eventually but plenty of people ignore them and nothing happens. Doesn't mean ignoring it is the right thing to do though.

Well you have to fill it out.
One of my friends husbands never filled it out and kept throwing it out until I’ve day a police officer showed up and demanded he fill it out or he’d get arrested.
It only takes a few seconds to fill it out. Worst comes to worse you might get called. But then maybe you can get an extension or a note from your work if they have very few employees or you might but get called.
Please just fill it out and not cause a chillul HaShem like that person did.

if you;re in brooklyn they have a huge pool. you have to fill it out or you can be in troubl. but the chances of getting on a jury are small.

You can not be imprisoned for not filling out the questionnaire. You can be improsined for not showing up to jury duty if you're called.
Op, you can ignore the questionnaire but they'll keep sending it and eventually you'll get a subpoena which will require you to go hand it in in person. If you don't you can get a fine (not sure about imprisonment).
Once you fill it out, if you do get called and you have a child under 2 or elderly parent that you are the primary caregiver for its very easy to get out of it.
Call the number, they'll tell you where to send the birth certificate or doctors note and you'll get a reply that you are exempt.

Huh? Did he get a summons or did the officer personally deliver it? Otherwise that makes no sense. If they send it via regular mail, there is no proof that you ever received it. They can't arrest you without any proof.

I know people who have been ignoring it for years on end. Unless it has been signed for, or if an official summons has been sent, they cant do anything about it.

I filled it out and then I got a notice to come to jury duty.I returned it with my Eleven year old's birth certificate and didn't hear back from them! I think I read until age 13 you can get away with it, as long as you don't work full time anyway.

Wow. For us they’re desperate. They only officially excuse for childcare reasons if you have a baby under 6 months.
