The top gates in Port Authority are not elevator accessible (yes I am sure) and I can't do escalators or large numbers of stairs. Is there any other way to get to Lakewood (and back..) directly from Manhattan?

There is a Monsey Trails bus from Brooklyn - stops in Boro Park and Flatbush. The question is can you get from Manhattan to Bklyn?

Yes they are elevator accessible. DH told me that when he takes the bus, he only uses the elevator as the doors open right by the departure gates. The escalator involves a lot of walking.

Also Academy bus goes from downtown Manhattan, picking up on the street, to Lakewood, though its only peak direction, meaning buses depart Manhatan between 2 and 815 PM, and depart Lakewood in the morning only until 750 AM

I checked that. Unfortunately their schedule is the opposite of what I need. They go FROM Lakewood in the morning (to take people to work) and TO Lakewood in the afternoon, starting at about 2 p.m.

Not always. I had an experience a few months ago were I showed up at PA, ready to go and the gate wasn't accessible. I went back to the ticket seller and he told me that they're "working on" having them all accessible by 2021.

Train from Penn Station to Point Pleasant. Bus to Lakewood. But its a roundabout way to go.

What does it mean the gate wasnt accessible? The elevator is there already. Maybe it was broken but otherwise how would they make it non accessible? Unless they meant that the bus itself isnt accesible, as you have to walk up 3 steps

The ticket seller said there was no elevator. That is all I know. I am sorry if this upsets you.

OP, the elevator may have been out of order that day. Or the ticket seller may have been mistaken, or have misunderstood your question. But the maps clearly show elevators to all levels of the Port Authority, and others have seen and used them.
Your best bet is to call the bus line and ask about wheelchair or handicap access to the area where the bus leaves. The Port Authority website also suggests, "We provide free assistance (including wheelchair escort service) for travelers who may have difficulty navigating their way through the facility. Simply visit our Information Booth or dial “54” on any in-house telephone to request staff assistance." That may be an alternative if you have difficulty.

I use the My MTA app and it has an accessibility tool where you can see accessibility of stations and even check elevator and escalator status.
If you don’t have the app, I think 311 can help with accessibility issues in public transportation. Is paratransit or Access-A-Ride an option? I’m not familiar with these myself.
