3.5 year old having accidents

My daughter has been completely toilet trained for 6 months. Now, all of a sudden she is wet all the time, except at night, when she is completely dry. It isn't like she has accidents because she is still making on the toilet, but every time she goes to make, I see that her underpants are totally wet. It is almost as if she starts to make in her pants and then makes the rest in the toilet. Anyone else have/ha this problem? Any advice?

My daughter still has this, and she is eight. She says that she doesn't realize that she needs to go until it starts to come out. We tried all sorts of positive reinforcement and going at certain times, but it didn't seem to help. Now we have her on medication, and it seems to be helping, except we're not very good about remembering every morning and every night. We've also noticed that she has a LOT more accidents/wet underwear, even dirty underwear during the week or two before she gets sick.
