You’ll have to find someone within your network, and it might just not be the “top” doctor you were recommended to. Unless you pay out of pocket

I would recomend you to reach out to refuah helpline or chaim medical for advice.
They might help you find a top dr within network and if thats a problem they can guide you through the insurance process

Unless a doctor is signed with Medicaid they cannot legally accept money from a patient enrolled in Medicaid.

You can, but it's going to cost thousands and thousands of dollars.
Instead of Medicaid you can buy a plan from the NYS exchange. You won't get any premium assistance because you are eligible for medicaid. The premium will be hundreds of dollars a month and the deductible will be thousands.
Or you can call a broker. I spoke with one recently who quoted me prices for cigna. Individual plan was over 800 a month and deductible was 5000, and after that is coinsurance.
How much are you prepared to spend on this?

Very hard if not impossible to find top doctors who take Medicaid. You get what you pay for. If your insurance is free you're likely not gonna get access to the best doctors

Columbia and NYU have excellent doctors for Crohns and they accept Fidelis and other MCD Hmos.

Typically a person needs a qualifying life event to even sign up, you can’t decide halfway through the year to switch insurance. Happens to be, now we have open enrollment and OP can apply (by 12/15 for coverage to start Jan 1st)
I did this for my last pregnancy, I’m on Medicaid but my dr wasn’t in network. I signed up for a marketplace plan as supplemental insurance and had zero credits towards the premium, so it cost me close to $900 monthly. I canceled it right after the baby was born. For me it was worth it
But as far as your situation, you can find a great doctor who accepts your insurance. Look up a reputable hospital’s recommendations for who’s in their network. It doesn’t need to be the most famous dr in NY

You have to tell them you don’t have insurance, once they know you have Medicaid they can’t legally charge you. To keep the costs down you’d need 2 doctors, one that takes Medicaid that will do your regular checkups, colonoscopies etc and then you consult with the top doctor on treatment options and general management.
