I had the same issue with one of my phones and it was actually a setting on my cell phone provider account. I have a Tmobile/sprint cell phone account with a few numbers and under my number there was a parental supervision or safe search setting that was clicked on. You can turn on and off this setting on the website.

Out of curiosity, when you're not on mobile data but on Wi-Fi, is the setting still on?
