Just one edit change יתגעגע to אתגעגע
Otherwise perfect
You can drop the word ani if you want, it's fine to leave it in as well

(you should) keep in touch = שמרי על קשר
Let's keep in touch = בואי נשמור על קשר/מקווה לשמור על קשר

Like to write this at the end with a phone number after it?

I would also add email address. Not every phone has messaging.

אליך is to a boy
אלייך is to a girl
