How old exactly? My just turned 4 year old loves to play memory. My 1 1/2 year old loves to make a mess of memory .

menchies, and all forms of pretend play - dolls, kitchen set etc. Keeps my kids (ranging from 2.5 to 10) busy for hours.

Organized group of toddlers. From 1 to 4. It needs to be an organized active game that looks professional. I've done duck, duck, goose and they don't really get it.

What about singing dancing games like monkeys in the bed, hokey pokey, wheels on the bus, etc....

from my experience of running toddlers service on shabbat, 1-4yrs are still a bit too young to play rule-based games like simon says or mr wolf. Agree with yo'ma 100% about sing-along with actions.
We used to do
sleeping bunnies ("See the sleeping bunnies, sleeping till it's noon...")
there's the synagogue version of Wheels on the Bus
("The curtains on the Ark go swish swish swish...")
And there's a nice pretend song that each children add "ingredient" to an imaginary pot to make chicken soup ("Put xyz in a pot, stir it well, make it nice...")
and of course there's the bracha tree song
