Sounds like nachos or mexican salad. Be more descriptive in your request and maybe we can help you. Was it a salad? Did it have whole corn chips. Did you eat in in a taco shell? Or maybe in a wrap or a flour tortilla?

Was it seasoned chopped meat on a bowl, with seasoned avocado guacamole on top, and then served with taco/tortilla chips on the side?

Its called mexican salad or Taco salad. There might be a recipe here for it. This is it loosely.
Layered in a bowl in this order:
Iceberg lettuce, shredded
red pepper, diced
cucumber, diced
tomatoes, chopped
avocado, cubed
can of corn, drained
scallions, sliced
On top of this, you pour a dressing made with mayonnaise, ketchup, lemon juice and Tabasco sauce.
On top of this you sprinkle chopped meat that has been cooked in an envelope of taco seasoning, according to pkg directions.
Sprinkle the edge of the bowl with crushed taco chips.
If you cant find the exact recipe, I can probably look it up for you.
Its a great salad for a warm Shabbos day.

I make a taco salad that sounds like that.
Veggies of your choice, lettuce, pepper, tomato, cucumber, whatever.
Ground Beef and crunched up taco chips.
Dressing - equal parts salsa and mayo, garlic powder. Mix, pour, toss, enjoy.

Buy Ortega taco seasoning packets, cook the ground meat according the the packet directions. (Or you can make your own seasoning mix if you can't find the Ortega stuff near you. I have a recipe.)
Make guacamole. Serve that on top of the meat and add some salsa as well.
Or maybe the salad was more like this. (You can just omit the cheese.)

lipid31, do you mash the avocadoes before mixing with mayo and salt? Or do you chop them and then mix them?

I've had a similar thing at someones house. It was sauteed chopped meat with onion and garlic. Then a layer of chopped tomatos (raw). Then a layer of salsa and mayo mixed together. Served with tortilla chips. YUM!
