If anyone can direct me as to how to obtain a w2 or w9 form from doe as a setts provider for 2019,

Contact aguda project learn. They will give you necessary forms to fill out. Norman rottenberg handles it at the city finance dept.

I have no idea what you're actually asking. W2 and W9 are two totally different things. They might ask you to send them a W9, which you can get by simply googling it. You don't get a W2 from the DOE unless they are your direct employer.

Tell them you want to be setss provider. They will give you all the forms. Hope that is helpful.
