Hatzalah of flatbush wouldn't take me to manhattan, only to Maimo
And like I said above, I was fine with it and I understood why they couldn't do it.
I think the policy is fair. The volunteers will take you to Maimo. If you want anything else they call a private company. And yes I did have a copay for it but it was my choice.

Ended up at their ER early last year and they botched things big time. Was admitted to Columbia with hardly any time to spare. Maimonides conveniently “lost” their records. It had been only a few days.
In general I’m all for form your own opinion, but when it can be dangerous, letoeles is in order. If you have the opportunity to go elsewhere, go elsewhere.
