I think it's from a time when you heated irons(made out of real iron, hence the name)on a fire and would use them for a little bit until they cooled down.
It means to do it right now don't procrastinate.

Ha! I was thinking of "that party on motzei Shabbat was off the hook!" (meaning it was very cool, and maybe a bit rowdy).
I assume comes from the idea of a phone ringing so much that the receiver is jostled out of its carriage.
