Every 3.5 years.
The one time we waited longer than 3.5 years, a piece of a letter had broken off.

Once upon purchasing, and then only when there is reason to think something may be wrong. Such as a flood or extreme weather event, the case cracked and weather could have gotten in, etc. Otherwise the usual practice nowadays is to wrap it in plastic so there's basically no wear and tear UNLESS you open it up, and it's better off being left alone.

Every three and half years is when we check.. here’s a spooky story—my husbands friend came to help put up our mezuzahs bc we got new ones.. they were hammering it in and then one of them dropped the hammer and then I heard a crash.. low and behold the mezuzah was glass and cracked and the case had been empty..
lesson learned- always check your mezuzah cases before you put them up..

I have heard of pple removing the mezuza once in 7 and having it checked twice....

Chabad. Our minhag is to check tefillin and mezuzos every Elul.

My brother-in-law is a sofer. He told me it's twice in 7 years. So he reminds his clients (he keeps a list of names and dates) every 3.5 years to get them checked.
