My 6 year old often compains of constipation and won't sit on the toilet for the time he needs (he's off and on off and on making a mess in the process)
I also find many times his underpants are dirty and he's smelly.
It doesn't seem to bother him.
what should I do? Is this something I should check by a Dr. as a medical issue?

give him something taht softens the stool. Definitely ask the Dr to look at it, and reward him for sitting. Read to him, give him books ONLY for toilet time, ec.

My brother has the same thing so I dont think its anything medically wrong. Maybe offer a prize if he sits on the toilet long enough.

miralax. Its over the counter and works great on young children. My son was on it for a while.
Also give him a lot of fiber, bran, apples with the peel, veggies, prunes (if you can get him to eat it.
