Question about tehillim

ok so I say tehillim for people I have in mind that need a refuah etc, but how do I really ahve them in mind? while I daven, is there a special soemthign to say before tehillim where I say the names of the cholim, how do all of you say it? I noticed they have all these tefillos for saying before and after tehillim, am I supposed to say them?

when I daven for people, I say "Hashem yishlach refuah sheleima, refuas hanefesh, refuas haguf l'___________ and then say whichever kapital of tehillim needs saying...

The exact text of a yehi ratzon you can say is usually under "refa'ainu" in shmoneh esrei in any siddur. There's usually a star, and then at the bottom of the page another star with the yehi ratzon in small text. I'd write it here, but I don't have Hebrew on my computer.
