whats the easiest way to get to and from these places?

My father is at einstein/montefiore hospital and I need to get to penn station

Take the "4" train heading "downtown" to "149th Street - Grand Concourse Station".
Transfer to the "2" train at 149th Street - Grand Concourse Station".
Get off at "34th Street - Penn Station".

You can get exact subway/bus directions from hopstop.com. The Albert Einstein hospital website also has pretty good directions. It doesn't look like it is too easy to get from one to the other though. The quick search on Hopstop I did came up with a 15 minute walk to the 5 train and you'd then have to transfer for the 2. Maybe spring for a cab if that is an option.
