I have done 2 layers of pajamas/sleepers when it's particularly cold since my kids are experts at kicking off the blankets.

Depends on how cold it gets. If it gets very cold, of course you put pjs on under it. Wash it same as pj's--after a week or when it gets peed, pooped or spat up on, whichever comes first.

For the last few weeks I put 2 layers of PJ's on my toddler. My son always kicks off his blankets and it's been getting pretty cold some nights.

Have you tried a baby sleep bag? (In America - e.g. Halo Sleepsack; in England - groBag or The Dream Bag or any generic one; or in Israel - The Dream Bag)

Babies can't kick it off at night and it can't go over their face, so it's much safer than blankets in any case. I use them for my kids here in Israel where inside the apartments it gets bone-chilling cold, much colder than outside and it works great!
