My neighbor who I am not so close with had a baby 3 months ago. She was not at home for the first month and was away when she did a kiddush in her home and I have just been super busy that I havent had a chance to bring something over and say mazal tov. we have such different schedules that I havent even seen her in the stairway. I am so embarassed!

is it really "better late then never"?

Absolutely. When would YOU rather get a mazal tov or a gift: late, or not at all?

I think now would be the best time.. after all she's had 3 months to settle down with a new baby and might appreciate a cake or something.

Go for it.

Just think how special it will be now. In the first few weeks, there's excitement and congratulations and presents. By 3 months, you're wondering if you'll ever be able to sleep, take a long relaxing shower, or use the bathroom in privacy. A new congratulations and a small present will be well-taken.
